Please complete the Student Co-op Evaluation form found here. You may return it to the Career Services Center in several ways:
1) Return it in person to RW 280;
2) Fax it to the Career Services Center at 216.687.9313; or
3) Email it to Yolanda Burt, Director, Career Services Center. Be sure to fill it out, save the document as "Co-op Eval for Your Name-Major-Co-op Semester-Year" and email it as an attachment to y.burt@csuohio.edu.
