There is a mandatory Health Services Referral requirement in the policy. This means that for all non-emergency medical care, you must first call and/or go to Cleveland State University's Health Service Center. If Health Services can not treat you, they will refer you to a doctor. A referral is not necessary under the following conditions ONLY:

1. Medical Emergency. The student must return to Health Services for necessary follow up care;
2. When Health Services is closed; (call Health Services the next business day to report that you went somewhere else for care)
3. When service is rendered at another facility during break or vacation periods;
4. Medical care is received when the student is more than 50 miles from campus;
5. When a student can no longer use Health Services due to a change in student status;
6. Annual PAP smear and maternity;
7. Mental health;
8. OPT students (Note: OPT students are not automatically billed for Health insurance. OPT students must complete an OPT enrollment for to be covered: SEE FORMS LINK ON MAIN PAGE).

In-network providers are covered at 100% up to the policy limits with a referral from Health Services (after your deductible is met).

Out-of-network providers are only covered at 60% up to the policy limits.

There is a $350 per policy year deductible on the health insurance policy.

There is an additional $125 co-pay for emergency room visits.

There is an additional $30 co-pay for urgent care visits.

There is an additional $20 co-pay for office visits.

There is an additional $250 co-pay for surgery.

Dental and vision are not included in your Health care plan.
