Any CSU community member that intends to drive a Viking Rental Fleet van or request the CSU business code for Enterprise rentals must become an approved driver by completing a Motor Vehicle Release and, if applicable, an online van safety training course. Fill out the form below to submit a Motor Vehicle Release and begin the driver approval process, or download a PDF form and submit it to transportation@csuohio.edu with a copy of your drivers license. Please allow up to 3 business days for processing. 

Please note, supervisor/advisor/department head approval is required to rent vehicles for official CSU business. Upon completing this form, an email will be sent to the listed supervisor/advisor/department head with instructions for email approval. Sensitive information, such as driver's license information, address, birthdate, etc., will not be sent to this individual, and will only go to PTS staff.
