Qualtrics Registration

Faculty and staff are responsible for general oversight of acceptable and responsible use of Qualtrics among students conducting research under their supervision. Students needing access should have their instructor or advisor request access via the form below.

Faculty Instructions: After you log in, please enter the student's CSU ID, first name, last name, email address, and last day of access to Qualtrics in the form below. You will receive an update about their account status within 2-3 days.

Bulk Form? If you are a faculty member trying to register multiple students at once, or if you are having difficulty with the form, you may contact c.pokorny@csuohio.edu to register them. Be sure to provide all of the following information, preferably in five columns in Excel: CSU ID, first name, last name, email address, and last date of access.
